Requirements for Visa Issuance Number Holders




1. Application form
2. Original Passport
3. One piece of passport size colored picture (Please attach on the application form)
4. Photocopy of Passport Bio-page (page 2)
5. Visa Issuance number (should be written on the application form)

6. Additional Documents

 - F1 : Photocopy of Employment Contract

 - E6 : 1. Photocopy of Employment Contract 2. Photocopy of Korean Company Business Permit 3. Photocopy of Certificate of Qualification or Experience(copy of document submitted during application of visa issuance number)

 E9, E10 :  1. NBI Clearance(issued within 3 months) 2. Health and Mentality Statement (Fill up attached form)


■ Visa fee: Php 2,000 (90 days single visa), Php 3,000(more than 90 days single visa), Php4,500(multiple visa)

* For E-9, please inquire at Philippine Overseas Employment Administration(POEA)


All applicants of long-term visa with the below nationalities are required to submit Certificate of TB (tuberculosis) Screening issued from hospitals designated by the embassy.

Angola, Azerbaijan, Bangladesh, Belarus, Cambodia, China, Congo, Ethiopia, India, Indonesia, Kazakhstan, Kenya, Kyrgyzstan, Laos, Malaysia, Moldova, Mongolia, Mozambique, Myanmar, Nepal, Nigeria, Pakistan, Papua New Guinea, Peru, Philippines, Russia, South Africa, Sri Lanka, Tajikistan, Thailand, Timor Leste, Ukraine, Uzbekistan, Vietnam, Zimbabwe.


** TB treatment history, Signs & Symptoms suggestive of TB, and Chest X-ray result are mandatory from Dec. 1, 2022 (Use attached format) 


  - Validity of the medical exam is 3 months

  - Children below 5 years old are exempted.
 

*Passport sized picture of the applicant should be attached at the Certificate of TB (tuberculosis) Screening. The picture should bear the dry seal of the hospital or the signature of the doctor in-charge. The laboratory test result should be submitted together with the Certificate of TB (tuberculosis) Screening.


In case the result of examination is positive, the visa application is automatically denied.
